I am thinking the root rot must be coming from contamination of the nutrients or the water, root rot doesn’t just magically appear and should never occur in a sterile environment, your roots should look like this…
I would throw away whatever nutrients your using and sterilize everything. Also bigger containers work better because it takes a lot longer for temperatures to effect larger volumes of liquids.
